The administrative staff, in the Port Saint Lucie office are involved in the every day operation of the office.  They manage the filing system and support the staff as needed through all administrative services.  Within this section you will find links to forms, applications, and statutes that staff frequently use and refer to.

For file reviews we suggest that you fax a request to (NAME AND FAX) stating what files you are interested in reviewing.  Please provide the following information: name of facility; address of facility; type of facility; county; and a date/time when you would like to review the files.  This is to be sure that we have the file(s) available for you when you come in.
